All too often children and adults with chronic disabilities are cared for by multiple specialists who do not communicate with one another. The unfortunate result – recommendations are made in isolations without the level of communication that is essential to a coordinated care plan. This can have a negative impact on care. Function brings our group of experienced specialists to a common site where each evaluates an individual’s problem(s) and then discusses management options in a group setting on the same day as the evaluation. The result is a coordinated treatment plan for the disability with the added advantage of a “one stop” assessment by all relevant medical specialties. We apply this model to all disabilities that result in neuromuscular, skeletal and urological dysfunction or anomalies.
