The Carl Sagan Discovery/Explainer Program

Navigating healthcare can be stressful—especially when your child has multiple medical conditions or requires medical equipment, such as feeding tubes or ventilators.

The LINCS (Linking Individual Needs of Children with Services) Complex Care Program can help. We provide family-centered, comprehensive care to children living at home with multiple conditions and chronic illnesses, from infancy to 21 years, and requiring medical equipment such as breathing machines, feeding tubes and wheelchairs.

The LINCS team works to provide coordinated and expert care for medically fragile children who need complex care, in close collaboration with:

  • Pediatric and surgical specialists within our hospital
  • Outside home nursing agencies
  • Regional and community pharmacies
  • Local programs that provide non-medical home services
  • Home hospice programs
